The con­cept of Pri­ma­ry Health Care (PHC) has been a cor­ner­stone in the pur­suit of Uni­ver­sal Health Cov­er­age (UHC) and Sus­tain­able Devel­op­ment Goals (SDGs). How­ev­er, amidst the evolv­ing land­scape of health­care, the true essence of PHC often gets lost in trans­la­tion. While the World Health Orga­ni­za­tion (WHO) and UNICEF offer a com­pre­hen­sive def­i­n­i­tion, the exe­cu­tion of PHC remains a chal­lenge, espe­cial­ly in ensur­ing equi­table dis­tri­b­u­tion and address­ing the broad­er deter­mi­nants of health.

In the 21st cen­tu­ry, PHC tran­scends mere med­ical inter­ven­tions; it embod­ies a holis­tic approach encom­pass­ing social jus­tice, equi­ty, and com­mu­ni­ty par­tic­i­pa­tion. Yet, the gap between vision and real­i­ty per­sists, demand­ing a par­a­digm shift towards health sys­tems designed for peo­ple, with peo­ple. Gov­ern­ments world­wide must pri­or­i­tize a whole-of-gov­ern­ment approach, embed­ding health-in-all-poli­cies to achieve UHC and SDGs effec­tive­ly.

PHC’s sig­nif­i­cance lies in its abil­i­ty to pro­vide whole-per­son care, address­ing not just phys­i­cal ail­ments but also men­tal and social well-being. It serves as the “front door” of the health sys­tem, offer­ing com­pre­hen­sive ser­vices from pro­mo­tion to pal­lia­tive care. How­ev­er, amidst the COVID-19 pan­dem­ic, the resilience of PHC is put to the test, high­light­ing the urgent need for invest­ments and reforms to strength­en health sys­tems glob­al­ly.

The WHO response under­scores the impor­tance of reori­ent­ing health sys­tems towards PHC to achieve UHC and SDG3. This involves not only strength­en­ing gov­er­nance and financ­ing but also pri­or­i­tiz­ing mar­gin­al­ized pop­u­la­tions often left behind.
